What You Will Do

As an Advanced Practice Provider (APP) for the Remote Patient Monitoring (RPM) team and Digital Clinic, you will be responsible for supporting the RPM Registered Nurse Navigators and the RPM population of patients.  Primary responsibilities include responding to escalations from the RPM Nurse Navigators and managing patients through virtual visits.  Escalations may include management of uncontrolled blood pressure with an adjustment in medications, management of an abnormal blood glucose by adjusting diabetes medications and/or evaluating the patient for worsening symptoms of congestive heart failure.  Medication refills, referral requests and other primary care responsibilities may also be required if the patient has not yet established with a PCP. Experience in chronic disease management, primary care and complex patients of varying socioeconomic backgrounds are skills required of this position.

The Digital Clinic or virtual care clinic for urgent care is also covered by the RPM APP so applicants for this position must also have experience in the care of minor illnesses.  Care is provided during a designated shift but on an on-demand basis, so comfort with managing multiple different tasks and time management are positive qualities for this position. This position is eligible for flexible work arrangements (telecommuting) for up to, but no more than 3 business days per week; a minimum of 2 business days per week must be onsite at the 7701 Metropolis Dr.



  • Obtains patient history and performs assessment via observation, interview and examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screening, immunizations and patient care.
  • Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.
  • Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources.
  • Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

What You Will Need


  • Required Credential(s):
    • Nurse Practitioner credentialed from the Texas Board of Nursing .  
    • Licensed Registered Nurse credentialed from the Texas Board of Nursing .  
  • Preferred Credential(s):
    • BLS Provider credentialed from the American Heart Association (AHA). 
    • Drug Enforcement Admin credentialed from the Drug Enforcement Administration. 


  • Master's degree of Nursing required


Work Experience:

  • 2 years direct patient care experience with complex chronic disease management preferred
  • Experience with telehealth/telemedicine preferred
